MI Choice Waiver (Calendar Year 2024)

For low-income adults who need nursing-level care in their home of choice. Milestone can help you or a loved one receive nursing services from home, even when income is limited.

About Waiver

Each MI Choice Waiver Participant has an individualized care plan outlining the services provided to them including the days, time and frequency of each service, if applicable. Services are funded through MDHHS for Medicaid eligible participants at no cost to the individual. Referrals for MI Choice Waiver services come from a variety of sources including potential participants and their supports, local medical providers, and partner agencies. Eligible services are provided up to seven days a week for participants based on their individual care plan. Milestone Senior Services contracts with local providers who furnish approved services to eligible participants.

Do I Qualify?

  • Seniors age 65 years or more
  • Persons with disabilities age 18 or more
  • Financially eligible for Medicaid
  • Meet annual income and asset limits set by the Federal Government
  • Medically eligible for long-term care
  • Must require one of the Waiver Services listed below
  • Must live in one of the following counties: Kalamazoo, Barry, Branch, Calhoun, or St. Joseph

Pesonal Emergency Response System

Milestone Senior Services offers a Personal Emergency Response System (PERS) service for people who are at high risk for falling and may not have someone at home to assist them. When activated by the client, the PERS device automatically dispatches emergency medical assistance to check on the client and evaluate their medical condition or needs. PERS provides a tremendous peace of mind for the people who have it and their loved ones.
