Over the past couple of years, Elaine has faced chronic health conditions and multiple hospital stays. After her fourth discharge, she was referred to Milestone Senior Services for the MI Choice Waiver program, which helps Medicaid-qualified individuals remain at home instead of moving to a nursing facility.
Milestone’s MI Choice Waiver social worker, Mallory, quickly set up in-home services for Elaine, whose husband works nights and can’t assist her during the day. These services included cleaning, shopping, showering assistance, appointment transportation, and a frozen meal delivery service to ensure balanced nutrition. Mallory also arranged for a Personal Emergency Response System for added safety.
One of the most transformative changes was the construction of a ramp by Milestone’s Home Repair & Accessibility team, covered by the MI Choice Waiver program. Navigating the three steps to her apartment was previously a major obstacle for Elaine, who uses a rollator walker and oxygen. With the landlord’s approval, the team built a ramp, giving Elaine newfound freedom.
Elaine’s excitement was palpable: “I can sit outside whenever I want, enjoy the fresh air, and listen to the birds. The ramp has made such a big difference for me.”
Thanks to the MI Choice Waiver program and Milestone Senior Services, Elaine has regained her independence and the ability to enjoy life’s simple pleasures.
