MI Choice Waiver Program
About Waiver:
Each MI Choice Waiver Participant has an individualized care plan outlining the services provided to them including the days, time and frequency of each service, if applicable. Services are funded through MDHHS for Medicaid eligible participants at no cost to the individual. Referrals for MI Choice Waiver services come from a variety of sources including potential participants and their supports, local medical providers, and partner agencies. Eligible services are provided up to seven days a week for participants based on their individual care plan. Milestone Senior Services contracts with local providers who furnish approved services to eligible participants.
Do I Qualify?

Seniors age 65 years or more

Persons with disabilities age 18 or more

Financially eligible for Medicaid

Meet annual income and asset limits set by the Federal Government

Medically eligible for long-term care
-
Must require one of the Waiver Services listed below

Must live in one of the following counties: Kalamazoo, Barry, Branch, Calhoun, or St. Joseph
