When it comes to connecting with health and care providers, sometimes finding the help you need is the hardest part. Matching your specific needs and circumstances to the requirements of various providers can feel like a never-ending game of go-fish. It can quickly become confusing and very frustrating.
The MI Choice Waiver program is tailored to provide in-home and community-based services for Medicaid-eligible older adults and adults with disabilities requiring services like those offered in a nursing facility. The MI Choice Waiver program supports independent living through coordination of services based on the participant’s individual needs, preferences, and goals.
Participants of the MI Choice Waiver program are partnered with a Registered Nurse and a Licensed Social Worker to develop an individualized care plan. Based on that plan, we assist participants in obtaining the in-home support they need to continue living in the home environment of their choice. Services are provided by a variety of sources including local medical providers, partner agencies, and the participants’ existing network.
MI Choice Waiver is a program of the Michigan Department of Health and Human Services administered by Milestone Senior Services. To qualify for MI Choice Waiver, participants must fall within income and asset limits set by Michigan Medicaid rules.
