Our Customer Service Team received a referral for Barb for the Mi Choice Waiver Program. Melissa from Customer Service attempted to reach Barb several times by phone but was unable to connect with her directly. She arranged to do a phone conference with Barb at her next appointment with the referring social worker and was able to obtain the needed information to move forward with the program application.
While talking with Barb, Melisa learned that part of Barb’s busy schedule includes participating in programming at the Ecumenical Senior Center. This turned out to be a very convenient coincidence because the Ecumenical Senior Center is currently operating at Milestone Senior Services’ facility while their new building is under construction. When Barb learned that “the helpful lady on the phone” was right in the building, she wanted to meet her in person. Melissa stopped by and visited with Barb between her poker games one day.
The MI Choice Waiver program connects people who are Medicaid-qualified to in-home services so that they can remain living at home with the supports they need. Each MI Choice Waiver participant has a care team consisting of a social worker and a registered nurse. Together, along with any family members or care-givers the client may have, an individualized care plan is developed and reevaluated as needed.
Melissa followed up with Barb sometime later to see how the MI Choice Waiver program was going for her and Barb reported that she is very happy with the service, and it is helping her remain independent in her home. Barb is very pleased with her Waiver team and is comfortable letting her team know if she has any concerns or needs any changes in her care plan. Barb said she highly recommends the Mi Choice Waiver program and is grateful for the help Customer Service and her Waiver team have given her at Milestone.
MI Choice Waiver is a program of the Michigan Department of Health and Human Services. Milestone Senior Services provides MI Choice Waiver programming for qualifying residents of Barry, Branch, Calhoun, Kalamazoo, and St. Joseph counties.
